By checking this box, I consent to the above mentioned individuals participating in the ICCP activities listed above. I hereby expressly release and forever discharge the Diocese of Joliet, Immaculate Conception Parish, and IC Catholic Prep, its trustees, officers, directors, employees, instructors, and staff, as well as any students or parents acting as such, from any and all liability, claim, loss, cost, or expense, and waive all rights and claims with respect to any and all personal injury or loss or damage to personal property arising out of such participation in the aforementioned activities. In addition, I understand and consent to the use of videotapes, photographs, digital images of above named individuals as they may be utilized by teachers, administrators, or their designees within the Joliet Diocese, Immaculate Conception Parish, and/or IC Catholic Prep for instructional and marketing purposes.
